Consensus: For healthy older adults with subjective memory decline, the strongest single trial is genuinely positive and the mechanism is coherent. Much of the older evidence base comes from stroke and dementia populations, where results are mixed — so the clean healthy-aging story rests mostly on one good study.
Where citicoline disappoints
- Acute stroke recovery — large trials (e.g. ICTUS) were negative; don't extrapolate the memory data to neurorehabilitation. - Young, healthy, unimpaired users — the memory benefit was shown in people with age-associated impairment; baseline matters. - Instant effects — this is a 12-week membrane-remodeling play, not a same-day stimulant.
What would change this grade. The tier is B because one high-quality healthy-aging RCT plus a coherent mechanism outweigh the mixed disease-population data. It would rise with independent replication in age-associated memory impairment, and fall if replications failed to reproduce the memory effect.
