What alpha-GPC does (and why it matters)
Acetylcholine (ACh) is the neurotransmitter of attention, working memory, and neuromuscular activation. Its synthesis is rate-limited by free intracellular choline at the presynaptic terminal, and cholinergic tone declines with age — the earliest, and most consistent, neurochemical signature of Alzheimer's-type cognitive impairment.
Alpha-GPC delivers choline more efficiently than any other over-the-counter form. Unlike choline bitartrate (poor CNS penetration) or CDP-choline (converts to cytidine + choline), alpha-GPC hydrolyzes to glycerophosphate + choline directly in the intestine, crosses the blood–brain barrier via a facilitated transporter, and is preferentially incorporated into phosphatidylcholine of neuronal membranes.

The GH-releasing effect (Kawamura 2012, PMID 22673596: 600 mg pre-exercise ~500% GH surge vs baseline in trained young men) is dose-dependent and short-lived. It's real, but do not use alpha-GPC as a "GH supplement" — the acute pulse doesn't materially shift 24-hour GH exposure.
When alpha-GPC is worth your money
Alpha-GPC earns a place in your stack when two or more apply:
- Age 55+ with subjective slowed processing speed or word-finding difficulty

- Building a nootropic stack around caffeine + L-theanine and need a cholinergic layer
- Recovering from head injury or on a cholinesterase inhibitor (physician-supervised only)
Skip or defer if you eat 3+ eggs daily (~750 mg dietary choline), take citicoline, or have never tested choline status and have no cognitive complaint.

Consensus: Tier B for cognitive support in populations with cholinergic deficit (AD, MCI, aging). Healthy-adult acute cognitive gains are real but modest and inconsistent. Longevity outcome data does not exist.
Where alpha-GPC disappoints
- Healthy young adults with normal diet. If your choline intake is adequate (eggs, dairy, meat), the acute cognitive delta is small and often unnoticeable. - As a "GH booster." The pulse is transient (~60 min) and doesn't move body composition on its own. - The 2021 stroke-risk signal. Lee et al. (PMID 33888053, JAMA Network Open) found a 43% higher 10-year stroke risk in Korean adults prescribed alpha-GPC — likely confounded (prescribed only to symptomatic patients), but the signal is real enough that anyone with vascular risk factors should discuss with their physician before starting.
