TL;DR — L-Arginine is the substrate for endothelial nitric oxide synthase (eNOS) — the enzyme that produces nitric oxide (NO) to relax blood vessels. Real RCT evidence for mild blood pressure reduction, endothelial dysfunction improvement, and erectile dysfunction. Post-MI use is contraindicated — the VINTAGE-MI trial (Schulman 2006 PMID 16391215) showed increased mortality in myocardial infarction survivors given arginine. Standard dose 3–6 g/day, but citrulline is often a better choice (higher plasma arginine per dose, better GI tolerance).
What L-arginine does (and why it matters)
L-Arginine is a conditionally-essential amino acid — the sole substrate for eNOS (endothelial NO synthase), which produces the ubiquitous vasodilator nitric oxide. Under normal conditions, dietary arginine plus endogenous synthesis from citrulline is sufficient; under stress (surgery, sepsis, immune activation), arginine becomes essential.
The rationale for supplementation is that: - Endothelial dysfunction (impaired NO production) is a hallmark of aging vascular biology - Aging arginase activity rises — arginase competes with eNOS for the same substrate, diverting arginine to urea rather than NO - Cardiovascular risk factors (hypertension, hypercholesterolemia, diabetes) all reduce NO availability
But the story has a twist: arginine at high dose can worsen outcomes in advanced cardiovascular disease. VINTAGE-MI showed post-MI supplementation increased mortality, likely because uncoupled eNOS in damaged tissue produces peroxynitrite rather than NO.

Chen et al. (1999, PMID 10404492) — chronic oral arginine improved erectile function in mild-moderate ED. Bode-Böger et al. (2003) — arginine reversed endothelial dysfunction in dyslipidemia. Menon et al. (2004) — post-MI infusion improved endothelial function. VINTAGE-MI (Schulman 2006, PMID 16391215) — 153 post-MI patients randomized to L-arginine 3 g TID × 6 months — trial stopped early due to 6 deaths in arginine arm vs 0 in placebo. Dong et al. (2011) meta — modest blood pressure reduction (~5 mmHg SBP). Citrulline supplementation (Ochiai 2012) may be preferable — better raises plasma arginine than arginine itself.

The VINTAGE-MI signal is the critical caveat: arginine in the setting of damaged, dysfunctional endothelium (uncoupled eNOS) can produce peroxynitrite instead of NO, worsening tissue injury. This is why arginine is not a routine post-MI supplement.
When arginine (or citrulline) is worth using
Arginine is worth thought when one or more apply:
- Mild-moderate erectile dysfunction (proven RCT indication)
- Endothelial dysfunction with normal LV function (adjunct)
- Modest hypertension not requiring pharmacotherapy
- Building an NO-supporting stack (usually citrulline is preferred over arginine)
Skip or defer if you're post-MI, have severe heart failure, active herpes flare (arginine can worsen), or expect large effects on athletic performance (evidence is mixed).

Where arginine disappoints
- VINTAGE-MI — post-MI arginine kills people; do not use in that context. - GI tolerance — high oral doses (>6 g) cause nausea, diarrhea; citrulline is much better tolerated. - First-pass metabolism — much of oral arginine is metabolized by intestinal arginase; plasma arginine rise is modest. - Herpes reactivation — high-dose arginine + low lysine can reactivate HSV in susceptible individuals.

Safety, red flags, and contraindications
- GI upset at doses >6 g common.
- VINTAGE-MI concern — do not use post-MI.

Do not self-start without clearance
- Recent myocardial infarction — VINTAGE-MI contraindication.
