Honest framing: Tier A for NAD+ elevation. No human longevity endpoint trial for either precursor. Mouse data supports both.
Where NR disappoints
- NAD+ goes up; function often doesn't. NR has more human RCTs than NMN — and that larger literature is sobering: several well-run trials raised NAD+ reliably but showed no improvement in insulin sensitivity, blood pressure, mitochondrial function, or body composition. The biomarker moves; the outcome frequently doesn't. - You may feel nothing. Because the functional payoff is inconsistent, many people see a NAD+ rise on labs and no subjective change — that's a legitimate result, not a dosing error. - It's redundant with NMN. NR converts to NMN en route to NAD+ — running both is paying twice for one pathway.
What would change this grade. NR is A for raising NAD+, not for clinical benefit. It would earn an outcome-level grade with a human trial showing a hard functional endpoint improves — the current, comparatively large NR trial base makes the absence of such a result notable. It would fall toward B for practical use if further trials keep showing NAD+ rises without functional gains.
