Correcting a genuine deficiency is clearly beneficial; adding selenium on top of an already-replete diet is not. What selenium does (and why it matters)
Selenium is not a molecule your body burns for antioxidant effect the way it does vitamin C — it is a structural cofactor. It gets incorporated as the amino acid selenocysteine into roughly two dozen human selenoproteins, and it is those enzymes, not free selenium, that do the work. The two best-characterized families are the glutathioneYour body's most abundant internal antioxidant, made from glycine, cysteine, and glutamate. The key biological fact that shapes the entire evidence base: selenoprotein synthesis saturates. Once you have enough selenium to fully express GPx and TrxR, additional intake does not buy additional enzyme activity — it just raises the toxicity risk. That saturation is why "correct a deficiency" and "supplement a replete person" are two completely different interventions with opposite risk/benefit.

The mechanistic role is not in doubt: selenoproteins including glutathione peroxidase and thioredoxin reductase are core redox enzymes whose activity is selenium-dependent (Handy & Loscalzo 2020, PMID 32380763, Antioxidants [Basel] 9(5):451). What is in doubt is whether supplementing beyond dietary sufficiency helps. The Selenium and Vitamin E Cancer Prevention Trial (SELECT) randomized 35,533 men and found that selenium "did not prevent prostate cancer" at a median 5.5 years (Lippman et al. 2009, PMID 19066370, JAMA 301(1):39–51). The trial was stopped early for futility, and its selenium arm contributed to concern about a diabetes signal.

The redox roles of GPx and TrxR are well-established biochemistry (PMID 32380763). But because these enzymes are the endpoint, the mineral behaves like a threshold nutrient, not a graded antioxidant: below sufficiency, adding selenium raises enzyme activity and measurably reduces oxidative damage; above sufficiency, enzyme activity is already maxed and extra selenium accumulates without benefit.
When selenium is worth your money
Selenium earns a place in your stack in a narrow set of cases:
- Documented low selenium status (low dietary intake, low-selenium soil region, or a low plasma/serum selenium lab), or
- Diagnosed Hashimoto's/autoimmune thyroiditis, where selenium has modest antibody-lowering evidence (see Section 3).
Skip it if you eat Brazil nuts, seafood, eggs, or organ meat regularly, take a multivitamin containing selenium, or have no reason to suspect deficiency — you are likely already replete, and the trials say replete people get no upside and a possible diabetes downside.

Consensus: Tier B — honestly split. The mechanism is solid (Tier A biochemistry), and correcting deficiency is well-supported. But the flagship supplementation trials are null-to-harmful: SELECT found no cancer benefit in mostly selenium-replete American men, and both the Stranges RCT and the Vinceti meta-analysis point to increased type 2 diabetes risk. The one area with genuine supplementation upside is autoimmune thyroiditis, where meta-analysis shows lowered thyroid antibodies — but even there the clinical durability is debated.

Do not self-start high-dose selenium
- Type 2 diabetes signal — a 200 µg/d RCT reported HR 1.55 for incident diabetes (PMID 17620655), and a meta-analysis found increased risk across exposure levels (PMID 29974401). Diabetic and prediabetic users should not supplement without physician oversight.
